The emphysema response to forced straining (Valsalva's maneuver).

چکیده
R ECENTLY there has been a renewed interest in the physiologic effects of forced straining (Valsalva's maneuver). Particular emphasis has been placed on the blood pressure changes during and immediately after forced straining in an attempt to define more accurately the diagnosis of certain circulatory abnormalities, particularly hemodynamically significant mitral stenosis and the presence of congestive failure.1-7 Little attention has been given to the manner in which pulmonary disease modifies the response to straining. The purpose of this paper is to present data obtained from observing circulatory changes during and immediately after Valsalva's maneuver in patients with pulmonary emphysema and to contrast these changes with responses in patients with normal and diseased hearts. The "emphysema response" is unique and to our knowledge has not been described except in our recent preliminary report.8

متن کاملA Case of Spontaneous Pneumomediastinum with Subcutaneous Emphysema in Children
Spontaneous pneumomediastinum is defined as free air or gas contained within the mediastinum, which almost invariably originates from the alveolar space or the conducting airways. It is rare in pediatric patients; however, occasional cases are reported to result from forced Valsalva's maneuver due to cough, emesis, a first attack of wheeze, or asthma exacerbations. We report the case of a 7-yea...
متن کاملAssessment of beat to beat changes in cardiac output during the Valsalva manoeuvre using electrical bioimpedance cardiography.
Beat to beat changes in cardiac output during standardized Valsalva's manoeuvres were recorded using electrical bioimpedance cardiography in 13 normal subjects. Cardiac output increased by 12 +/- 5% after 1 s of straining solely because of an increase in heart rate. Subsequently, cardiac stroke volume and cardiac output fell during the strain to lows of -40 +/- 6% and -21 +/- 3% respectively at...
متن کاملSpontaneous pneumomediastinum, pneumoretroperitoneum, and cervicofacial subcutaneous emphysema after repeatedly and forcefully blowing into a bottle
Spontaneous pneumomediastinum (SPM) is an uncommon, self-limiting condition associated with increased intra-thoracic pressure resulting in alveolar rupture. Search of the literature revealed no detailed case report about a 26-year-old psychiatric patient who repeatedly and forcefully blew air into a bottle for 5 days resulting in a combined condition of spontaneous pneumoretroperitoneum, pneumo...
متن کاملEF FECTS OF THE VALSALVA MANEUVER AND HEAD ROTATION ON INTERNAL JUGULAR VEIN DIAMETER AND LOCATION BY ULTRASONOGRAPHY
In order to determine the effect of Valsalva maneuver and head rotation on the internal jugular vein (IJV) diameter and cross-sectional area and overlap with the carotid artery (CA) and find the best technique for safe cannulation of the IJV and decrease the risk of CA puncture, the diameter of both the IJV and the CA and percentage of overlap between the two vessels and cross-sectional are...
